Gender-related controversies in employment, healthcare, sports and law have remained at the forefront of many news stories in 2024. As far as the mental health professions are concerned, we saw the release of the final report of the Cass Review in April 2024, its endorsement by responsible government agencies in the UK, as well as some push-back from activist physicians and pressure groups.

One such response was by activist clinicians and scholars who are best known for their pro-medicalisation position on transgender health, minors included. Under the “Integrity Project” of Yale Law School, they published their “Evidence-Based Critique of the Cass Review.” Although long, we encourage our readers to look at this report.

The organisation, Democrats for an Informed Approach to Gender (DIAG) critically analysed this report, and found several major drawbacks in the report, stating the following:

“The report is non-peer-reviewed, and even though they do not declare any conflict of interest, some of its authors have published their own studies that were rated poorly by the Cass Review.

“The Yale report argues that the Cass Review overlooks positive outcomes of GAC, such as improved body satisfaction, appearance congruence, quality of life, psychosocial functioning, and mental health, as well as reduced suicidality, providing as evidence two studies: Chen et al. (2023) (the “longest and largest study to date”) and Tordoff et al. (2022). The report also claims that the hormone treatments lead to improved mental health by targeting appearance congruence. We test the validity of these claims by reviewing these studies in detail.”

This critique is a fascinating read, indicating the extent to which those who are ideologically committed are prepared to go to prove what they set out to prove. The full critique of the Yale Integrity Report can be found here.
